An LPA is a legal document which allows a person who is at least 21 years of age ("Donor"), to voluntarily appoint one or more persons ("Donees(s)") to make decisions and act on his/her behalf if he/she loses mental capacity one day.
You can make your LPA using the Office of the Public Guardian (OPGO) Online system. OPG lets you book an appointment with a participating CI using the Health Appointment System (HAS). Booking on HAS is free, and also shows you the cost of LPA certification at the clinic.
The CI witnesses and certifies the LPA by ensuring the Donor understands the purpose of the LPA, the authority they are giving their Donee(s), and that they aren't being pressured or defrauded into making the LPA.
